Although nobody likes to see a failure, especially a catastrophic one, it is important to learn whatever we can from them so that the geo-community does not make the same mistakes again.

The Mill Creek Dam near Walla Walla, Washington has served its purpose admirably for over 67 years but according to a recent report by the US Army Corps of Engineers, the dam received a Dam Safety Action Category (DSAC) rating of 1 out of 5 which means that the risk to public safety is too great when Bennington lake which is formed by the dam is more than 17% full for an extended period of time. In case you missed the background on the Kilbuck Landslide (or here or here), it happened in Kilbuck Township, PA back in in September of 2006. To deal with the political hot potato and media nightmare, Wal-Mart bought out the developer on the unfinished property and began handling remediation of the slide itself. Now, it appears as if they want their money back from the developer! Read on for more details.
According to Silicon Valley’s MercuryNews.com, a [very lucky] worker suffered minor injuries when the crane he was opperating toppled down an embankment. The accident apparently occured on a CALTRANS project as the crane was lifting a 10,000-lb pile when a "wood retaining wall" gave way. A 12m long sinkhole opened up on Oxford Road in northern Johannesburg South Africa after a partial tunnel collapse in the Gautrain rail tunnel being constructed underneath the road. Eyewitness accounts say there was a broken water pipe flooding the sinkhole, but no word on which occurred first. The road is expected to be closed for 2 weeks. Gautrain representatives said the tunneling would resume after geotechnical/geological investigations into the collapse are completed, which could take "several weeks". On May 15 a drilled shaft rig operated by Mid America Drilling Corp was nearly swallowed by the 9-ft diameter hole it was drilling for a cell tower foundation near Harlan, Iowa. Fortunately nobody was injured and the truck did not suffer any significant damage.
In Arizona, the standard of practice is to use a temporary surface casing to prevent dangerous failures like that. I once saw pictures of a drilling contractor superindendent almost get sucked into a collapsing shaft, even with the temporary casing. Pretty scary stuff. Source: Harlan Tribune. The Countywide Recycling & Disposal Facilities in Stark County, Ohio have a new round of problems. There has been much debate over an underground fire reportedly caused by the disposal of aluminum dross waste, a byproduct of aluminum production, dumped between 1993 and 2001. The site has long received complaints for odors and for a long time denied that there was a fire. They have also had problems with slope stability, including a failure of a portion of the south slope in 2006, possibly a result of the destabilizing effect of the fire. Early this year the Ohio EPA cited the facility for violating 8 environmental regulations after waste was found to have migrated outside the permited waste area.
Their latest problems involve a tear or leak of a geosynthetic lining that caps the facility to control gas and odors from the underground fires and regular landfill gas. The leak resulted in 500 to 5,000 gallons of leachate spilling into a sediment pond. The material fortunately did not leave the site and was pumped out of the pond and transfered to a treatment plant where their leachate usually goes. The landfill, one of the largest in Ohio, handles about 6,000 tons of trash a day, including about half of Summit County’s residential garbage.

The City of Vancouver is suing a developer, excavation contractor and their consulting engineer for the costs of repairs, overtime for city employees and lost revenue from parking meters etc stemming from an apparent failure of a shoring system that formed a 30-meter sinkhole. No mention of the developer’s name or the engineer, but the contractor was Matcon Excavation and Shoring. The site will be the future home of high-rise condominiums…if the City lifts it’s stop work order.
The failure of the shoring caused a break inf a 20-cm water main ultimately flooding the site. It also necessitated the closure of the adjacent street. Of course this invites the whole chicken or the egg scenario. The defendants will probably argue that the water line failed first causing the failure of the shoring, but of course the City Engineer, Tom Timm was not shy about fingering the shoring as being deficient.
"It’s some kind of a failure of the shoring system . . . either a design issue or the way it was put in place."
On December 14, 2005, the upper reservoir of the Taum Sauk pumped storage plant, a hydroelectric power plant in the Missouri Ozarks, suffered a catastrophic failure. The resulting flood severely damaged the Jonhson’s Shut-Ins State Park and swept away the park superintendent and his family. Fortunately all of them survived. Last week, the Missouri Attorney General’s office announced a settlement of $180 million between the facility’s owner, Ameren Corp. The failure reportedly was a result of negligence on the part of the company. More details after the break.
[Update 12/6/07] San Diego City Council approved $20 Million for the repair of Soledad Mountain Road despite concerns by some about whether funds diverted to pay for the fix will ever be repaid by Federal and State monies. Additionally, shear pin installation mentioned on the next page is scheduled to be completed by Saturday. [/Update]
